WAIVER, RELEASE AND CONSENT TO TATTOO

I confirm that I am over the age of 18, will provide valid photo ID, and I give permission to Princelet Tattoo London and the working artist to tattoo me.
I understand a tattoo is permanent and it is at the tattooist's discretion to refuse anybody’s custom and that all deposits paid are non-refundable under any circumstances.
I am not under the influence of alcohol or drugs, and I am voluntarily submitting to be tattooed by the Artist without duress or coercion.
I have been fully informed of the inherent risks associated with getting a tattoo. I understand that these risks, known and unknown, can lead to injury, including but not limited to infection, scarring, difficulties in detecting melanoma, and allergic reactions to tattoo pigment, latex gloves, or soap. Having been informed of the potential risks associated with getting a tattoo, I still wish to proceed and I freely accept and expressly assume all risks.

I approve that I have checked the design, spelling, and all details. Any mistakes (judging by the prepared stencil and design) are not the artist's fault. If corrections are needed, they will be done at a separate cost.
 
I do not have a heart condition, nor do I take blood thinning medication. I am not pregnant or nursing. I do not have any other medical or skin condition that may interfere with the application or healing of the tattoo. I am not the recipient of an organ or bone marrow transplant, or, if I have been, I have taken the prescribed preventative antibiotics required by my doctor. I do not have a mental impairment that may affect my judgement in getting the tattoo.
If you have suffered, are suffering, or are receiving treatment for any of the following illnesses or conditions, you must notify your tattooist. Required
I also confirm that I will follow the aftercare procedure until healing is complete. I understand that notwithstanding the hygienic conditions and sterile instruments used by this tattoo establishment, there are risks associated with tattoos which include scarring, allergic reaction, and localised swelling.
 
I understand that failure to follow aftercare advice may cause complications and will void my right to a free touch-up. Any touch-ups needed due to my own negligence will be charged at the standard rate.

In giving this consent I release this tattoo establishment and its employees from all liabilities, actions, and demands which I may have now or in the future for any loss or damage suffered, however caused, as a result of my tattoo or in respect of personal injury caused by the negligence of the establishment and any failure on my part to follow the aftercare procedures.

Variations in colour and design may exist between the tattoo art I have selected and the actual tattoo when it is applied to my body. I also understand that over time, the colours and clarity of my tattoo will fade due to unprotected exposure to the sun and the naturally occurring dispersion of pigment under the skin.

The artist and the Tattoo Studio have given me instructions on the care of my tattoo while it is healing, and I understand them and will follow them. I acknowledge that it is possible that the tattoo can become infected, particularly if I do not follow the instructions given to me.
 
I confirm that I am not pregnant, not attempting to become pregnant, and not currently breastfeeding (nursing).
 

Guest Artist Disclaimer

From time to time, Princelet Tattoo London hosts guest tattoo artists who are independent professionals. While we carefully select the artists we collaborate with, we do not assume any responsibility or liability for the outcome of tattoos performed by guest artists. Any issues regarding healing, touch-ups, corrections, or changes must be addressed directly with the guest artist. Princelet Tattoo London does not offer or cover the cost of touch-ups or alterations for tattoos completed by guest artists under any circumstances.

I release all rights to any photographs taken of me and the tattoo and give consent in advance to their reproduction in print or electronic form. (If you do not agree, please advise your artist and the Tattoo Studio before your tattoo begins.)

A tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which may be disfiguring and costly, and may not restore my skin to its original appearance.

I am aware that hands, fingers, feet, and lower ankle tattoos are prone to ink not holding well in the skin. These areas may require additional work and are charged at the studio’s standard tattoo rate.
